上海市崇明区港沿镇社区卫生服务中心位于崇明岛中部，建筑面积5117平方米。中心下设一个分中心，21个村卫生室。服务面积75.11平方公里，服务人口4.8万。中心内设全科、中医科、妇幼保健科、口腔科、预防保健科、卫生监督科、放射科、B超室、心电图室等门诊科室，拥有B超、心电图、X光机、全自动血球计数仪、康复器材等医疗仪器设备，核定床位40张，设有综合病房和老年护理病房。中心秉承优质服务理念，以保障社区百姓健康和提升社区百姓满意度为目标，积极探索开展家庭医生制签约服务等工作，全方位满足社区百姓的基本医疗和基本公共卫生服务需求，努力为社区百姓提供质优、价廉、温馨、便捷的医疗卫生服务，担当社区百姓健康“守门人”。
